2       Q Did Mr. Canada have a middle name to your

3  knowledge?

4       A I believe it is Craig.

5       MR. SASNETT: May I approach the witness your Honor.

6       THE COURT: Yes.

7       MR. SASNETT: I am going to show Deputy Dougan what

8  has been marked No. 1 for identification.

9  It is a document headed at the top, basically,

10 "Physician's Statement."

11      Q (BY MR. SASNETT:) Do you recognize what that is

12 a copy of?

13      A Well, I have seen -- I saw this at Mr. Canada's

14 house, several like it and this may be a facsimile of a

15 copy of that. I am not sure.

16      Q When you prepared your report, did you put a

17 copy of the physician's statement as well?

18      A I believe there are several in there.

19      Q Were they all similar?

20      A Yes.

21      Q And I am showing you another exhibit marked No.

22 2 for identification, also headed "Physician's Statement"

23 near the top.

24      I'll direct your attention to the obvious

25 different date towards the lower left-hand corner?

26      A Correct.

27      Q Other than that are they substantially similar
